Thank you for signing up for Myo DIY.

We look forward to providing you the highest standard of care!

Quick Overview :

Cancelations or reschedules within 24 hrs will incur a $50 fee. Reschedules made 25+ hours in advance are welcome.
Payments are billed weekly regardless of appointment date (ie, like a subscription.) Once Package amount is is paid in full, payments cease.
Myo DIY will not contact Client insurance for any reason. Client must facilitate all insurance information submissions personally.
